Our commitment is to respect and protect the privacy of all users of our website, as well as our valued existing and prospective clients. We are steadfast in preserving the confidentiality of your personal information throughout your engagement with our center while adhering to the legal standards that govern all mental health professionals. 

 

Collection and Utilization of Personal Information 

 

As providers of mental health services, we obtain personal information directly from you or individuals acting on your behalf. This information typically encompasses your name, contact details, address, and the underlying reasons for your request for our services. We collect this information solely to provide you with the services you require. Consequently, it is shared exclusively with authorized administrative personnel and potential therapist matches to best serve your interests. 

In addition to the information provided at the onset via the intake and consent to treatment form, your clinician will take progress notes and may prepare personal evaluations throughout your treatment. As per mental health professional order’s standards, all clinical notes including psychological records, appointment notes, treatment interventions and the results of treatment will be stored in your personal file housed exclusively on our electronic practice management software and will only be made available to those individuals who are required to access it. This will ensure that the standards of care continue to be met throughout and after the course of your treatment. 

 

 

Security Measures for Your Personal Information 

 

At Grey Zone Psychology and Wellness, any suspected privacy breaches are subject to immediate investigation and resolution to prevent any harm to our clients and community. All members of the Grey Zone Psychology and Wellness team are obliged to sign a confidentiality and privacy agreement, underscoring their commitment to protect your personal information and utilize it solely for facilitating your access to mental health services. 

 

Deletion and Elimination of Your Personal Information 

 

If you wish to have your information removed from our systems, a written request to us will trigger the deletion of all your personal data within 72 hours. In the absence of such a request, your personal information will be retained for the necessary duration to provide you with services or as mandated by the laws and regulations governing our practice. The disposal of private information adheres to secure and confidential protocols, aligning with the requirements of our affiliated governing bodies. 

Sometimes, we may collect personal information to send you emails containing our latest news or services You can unsubscribe from our mailing list at all times by following the instructions provided at the end of every email.  

As per standards set forth by the OPQ, the Clinic is responsible for the safeguarding and storage of all client-related information for a period of 5-years from the date of the last professional service provided. Subject to this requirement, all clinical information including any physical copies may be destroyed/shredded only after this 5-year period in order to ensure the protection and confidentiality of all clients. 

 

Sharing of Your Personal Information 

 

We will not disseminate any information you share with Grey Zone Psychology and Wellness to third-party entities without your explicit written consent. We will not share your information with third parties for marketing purposes. We maintain an email list to keep subscribers informed about upcoming groups and workshops, and we provide an unsubscribe option for our email newsletter. 

 

Requests for Your Personal Records & Collaboration with Third Parties 

 

Under certain circumstances, you may wish to access your personal records. In such instances, your therapist will evaluate which information can be disclosed in accordance with their governing regulations. Similarly, if you desire to grant access to your personal records to a third-party healthcare provider for collaborative care, this can only occur with your informed written consent.
